Permanent, Part time (2 x 12 hour shifts on a Rota basis).

£50,000-£55,000 Pro Rota (Depending on experience) plus an amazing benefits package!

We’re looking for an experienced and forward-thinking Advanced Clinical Practitioner (ACP) to join our dynamic Digital Care Team. This is an exciting opportunity to play a key role in shaping the future of healthcare delivery through innovative digital solutions.

You’ll collaborate with Specialist Registrars, Consultants, and a multidisciplinary team, working at the forefront of digital healthcare innovation. • Proven experience working autonomously in an advanced nursing role.
• Demonstrated ability to assess, diagnose, and manage both acute and chronic conditions independently.
• Experience in prescribing medications and conducting medication reviews.
• Skilled in managing patients with long-term and complex conditions.
• Experience in nurse-led triage and case management.

📚 Experience We’re Looking For

• Registered nurse or other advanced practitioner with current NMC/HCPC registration
• MSc in Advanced Clinical Practice or equivalent postgraduate qualification.
