Permanent, Full Time
Monday - Friday with a Saturday shift twice a year
Homebased with office visits every 3 months
Are you passionate about delivering exceptional customer service? We’re looking for a Care Liaison Advisor to join our team and liaise with families and hospitals to triage care home placements and packages of care.
Acacium Group are a leading global healthcare delivery partner and we are hiring a Care Liaison Advisor. Part of Acacium Group, CHS Healthcare offer a personalised service that comes at no cost to individuals or their families. Our expert advisers are here to support and guide service users every step of the way. We spend time asking people to tell us a little bit about their needs, and our team will provide them with a tailored shortlist of care homes and care home availability in their area.
đź’ˇ About the Role
In this role, you will:
Provide exceptional customer service to patients and their families, supporting them during stressful times with sensitivity and empathy.
Accurately record data on our internal system when triaging referrals onto our internal teams.
Provide our internal teams with accurate detailed referrals and updates.
Act as the communication link between care homes, hospital discharge teams and patients' families to ensure everyone is updated.
This is a role where your exceptional communication skills will make a real difference.

📚 Experience We’re Looking For
Administration experience, updating data on internal systems and using Microsoft Office packages.
Customer service experience over the phone.
Experience in a healthcare setting.
