Shift patterns between 07:00-15:00 and 11:00-19:00 + 1 weekend every 6 weeks

We are hiring a Care Coordinator to join Thornbury Community Services, part of Acacium Group. Thornbury Community Services provides complex care services that enable people with healthcare needs to live independently and safely in their own homes.

In this role, you will work closely with clients, healthcare professionals, nurses and carers to ensure the delivery of safe, high-quality care. You will be responsible for coordinating care packages, managing staffing rotas, and acting as a key point of contact for clients and healthcare workers, ensuring exceptional service and continuity of care at all times

In this role, you will:

Coordinate and manage up to 7 complex care packages, ensuring high-quality care delivery.
Build strong relationships with clients, families and healthcare workers, acting as a key point of contact.
Act as a key point of contact for clients, families, carers, nurses, and stakeholders.
Manage staffing rotas and source healthcare workers to ensure packages remain fully staffed.
Support onboarding activities, including training, shadow shifts and compliance processes.
Maintain accurate records and documentation in line with regulatory requirements.
Collaborate with clinical and operational teams to resolve issues and improve service delivery.
Monitor package performance and implement solutions to maintain excellent client outcomes.

What we’re looking for:

Previous experience in a customer service, care coordination, scheduling or healthcare administration role.
Excellent written and verbal communication skills with the ability to build strong relationships.
Strong organisational skills with the ability to prioritise and manage a busy workload in a fast-paced environment.
Confidence handling high volumes of calls and responding to changing priorities.
A proactive approach, strong attention to detail and a genuine passion for delivering outstanding care and service.
