Are you a proactive and empathetic communicator with a passion for helping others lead healthier lives? We have an exciting opportunity to join our team as a Patient Coordinator, where you'll play a pivotal role in guiding individuals through transformative health and wellbeing programmes.
We're looking for a personable individual who is confident over the phone, as you'll be delivering high-quality support, conducting initial eligibility assessments and helping service users overcome barriers to participation. Your insights will ensure each individual is matched with the most suitable programme, and your ongoing support will help them stay engaged and empowered throughout their journey.

We work with service users to make the changes that stick. Our people will be with you every step of the way, translating the science into healthy habits that add up to long term health improvement.
Every day you will:
- Contact Service Users following receipt of their referral to promote the programme, offer support and resources, and deliver outstanding customer service
 - Conduct initial assessments during booking calls, including triage for further coaching calls where appropriate
 - Carry out routine and ad hoc administrative duties to support the smooth running of the service, including data entry, document management, and scheduling
 - Monitor and manage the shared mailbox, ensuring all incoming queries are acknowledged and responded to promptly and professionally
